Anticipatory care hub

Proactive health and care intervention at individual and population level. It is proactive care and support, targeted at people living with frailty, multi-morbidity and/or complex needs to help them stay independent and healthy for as long as possible at home or the place they call home focusing on what is important to the individual.

Children and young persons’ (CYP) wellbeing service

Supporting CYP’s and their families at an early stage to help them to navigate the system, and therefore prevent mental health conditions from escalating.

Among the problems the team can assist with are mild mental health problems, low mood, low self-esteem, bullying and behavioural problems. While they’re not able to offer counselling services they can offer a holistic approach and can provide valued support while children await referral appointments.

Engaging Minds Surrey

A group for those diagnosed with Dementia to socialise with others in a similar position.

Complete online referral form

GP integrated mental health service

Providing patients with quick and easy access to a range of mental health support and treatment at their local doctor’s surgery. The team will work with the patient to make a plan to get them the help they need and aim to see people within four weeks of being referred. Up to four appointments are usually provided which can be face to face, over the phone or by video call.

Growing Health Together

Growing Health Together invites people living and working in communities across East Surrey to collaborate, in order to co-create conditions in which everyone’s health and wellbeing can flourish. We’re proud founding partners for the initiative and are committed to ensuring that health and wellbeing exists outside of the formal health service, in the places we live. From walking groups, coffee mornings and mutual aid groups, through to our local parks, lakes and wild spaces, there is so much close at hand to support our wellbeing.

Out of hours appointments

We work in partnership with our GP federation Alliance for Better Care CIC to run the Enhanced Access programme which provides out of hours appointments for our patients.

This includes face-to-face and remote appointments on weekday evenings and Saturdays.

To book an out of hours appointment, patients contact their GP Practice.

Vaccination programme

We continue to work with our GP federation to deliver a safe and effective COVID Vaccination Programme to our community. The spring vaccination programme will begin on 15 April for care homes and housebound patients. All other eligible cohorts will be seen from 22 April.
